What is new this year?

Instead of the emailed survey used last year, you will now submit your data via a form available in your CareFirst Employer Portal.

What will I need to provide?

States where plan is offered
HMO plans – MD, DC, VA​
PPO & ADV plans – National
Your PBM is CVS Caremark – No need to enter PBM & EIN

The form will prompt you to select states where plan is offered to employees.

HMO plans: Select MD, DC, VA​
PPO & ADV plans: Select National

Next, you will provide two pieces of data.

Premiums paid by employee (payroll deductions)
Premiums paid by employer (total billed minus payroll deductions)
